APPOINTMENTS
If you cannot keep a scheduled appointment or will be delayed, please call us as soon as possible. We request 24 hours notice for cancellations.
INSURANCE & BILLING
We accept most traditional insurance plans, but please contact our office to verify acceptance of your plan. Payments and Copays are due at the time of service. We will bill your insurance, but it is your responsibility to be sure you are covered for the visit. We will charge a fee for paperwork such as FMLA, disability forms, etc.
PAYMENT OPTION
We accept checks, cash, or credit cards. Please contact our office for details.
